Position Overview

BayCare Medical Group is a physician-led medical group seeking a dedicated board certified Advanced Practice Provider (APP) to lead our Heart Failure Clinic. The ideal candidate will play a vital role in managing patients with heart failure, ensuring high-quality, patient-centered care while collaborating with a multidisciplinary team.

 

Key Responsibilities:

  • Patient Care: Conduct comprehensive assessments, develop and implement individualized treatment plans, and manage ongoing care for heart failure patients.
  • Education: Provide patient and family education regarding heart failure management, lifestyle modifications, and medication adherence.
  • Collaboration: Work closely with cardiologists, nurses, and other healthcare professionals to coordinate care and optimize treatment outcomes.
  • Clinical Procedures: Perform relevant clinical procedures and interpret diagnostic tests to guide patient management.
  • Quality Improvement: Participate in initiatives aimed at enhancing care quality and clinic efficiency, including data collection and analysis for ongoing research.
  • Documentation: Maintain accurate and timely electronic medical records to ensure compliance with regulatory standards.

Qualifications

Position requires: Florida licensure and certification/NP-C or PA-C.  

  • Master’s degree in Nursing or Physician Assistant Studies from an accredited program.
  • Current certification as a Nurse Practitioner (NP) or Physician Assistant (PA).
  • Valid Florida licensure to practice as a Nurse Practitioner or Physician Assistant.
  • Clinical experience in cardiology, heart failure management, or a related field preferred.
  • Strong clinical assessment skills and a comprehensive understanding of heart failure treatment protocols.
  • Excellent communication and interpersonal skills, with a passion for patient education and advocacy.
